For standard passenger cars, four inches over a compacted base is the usual spec. If you’re parking a truck, an RV, or heavy equipment, you want five or six inches with reinforcement. We confirm the thickness during the site visit, not over the phone.
Mostly freeze-thaw. Water gets into the concrete, freezes, expands, and pushes the surface apart. A poorly compacted base makes it worse. Control joints give the slab a planned place to crack instead of a random one, and correct thickness handles the rest.

Sometimes. If the base underneath is still solid and the damage is surface level, patching or resurfacing works and costs less. If the slab has settled or the sub-base failed, repair is money wasted. We’ll tell you which one you’re dealing with.
Spring through fall is easiest. Concrete needs temperatures above freezing to cure properly, so mid-winter pours need extra measures and cost more. If your project can wait for better weather, we’ll say so instead of pushing the job.
